Is it your mattress or your foundation?

Short answer. One useful screening check: place the mattress temporarily on a surface you know is flat, rigid, and fully supported, then compare two things — its unloaded shape, and how it responds where you actually sleep. If the problem largely disappears, the base is likely contributing. If it remains in the same area, the mattress is more likely contributing. If it improves but doesn't disappear, both may be involved. The check narrows the cause before you spend money on either side — it isn't a warranty measurement or a definitive diagnosis.

Before spending money on either a new mattress or a new frame, it's worth finding out which one actually needs attention. The two problems can feel very similar from where you're lying, and guessing wrong means paying for the fix that wasn't the problem.

Why this is hard to tell from the bed itself.

A mattress that's losing support in the middle feels soft and low where you sleep. A mattress sitting on a sagging box spring, a warped platform, or slats spaced too far apart can feel very much the same — because the mattress is following whatever shape is underneath it. From on top, a change in the mattress and a change in the base can produce a very similar sensation. That's the whole problem.

What's actually happening underneath.

Body weight concentrates in the center third of the mattress during sleep, making it the first area to lose structural support over time. That's true of the mattress's own support layer — but it's just as true of whatever is holding the mattress up. A center support beam without a leg underneath it, slats that have started to bow, or a box spring with broken internal webbing can all sag in that same center-loaded zone. The mattress isn't lying to you about what it feels; it's just reporting the shape of whatever it's resting on.

The known-flat comparison
  1. First inspect your current setup: frame, center support, legs, and slats, for damage or obvious bowing.
  2. Move the mattress to a known-flat, rigid, fully supported surface, and allow it to settle after moving before comparing.
  3. Compare the unloaded shape: does the same visible indentation remain? Then compare the loaded response in the same sleeping area: does the mattress still allow the same uneven settling under you?
  4. Read the result: largely gone — the base is likely contributing. Unchanged in the same area — the mattress is likely contributing. Improved but present — both may be involved: correct the base first, then repeat the comparison before deciding anything else.

This works because it removes the one variable you can't see by eye: whether the surface underneath the mattress is actually flat. A sagging base can hide a healthy mattress's true shape just as easily as a healthy base can expose a failing one.

The check has three possible outcomes. A partial improvement suggests that both the support surface and the mattress may be contributing.
The mattress reports the shape of what's under it. The comparison tells you whose shape it actually is.

Common foundation issues that mimic mattress sag.

A handful of base problems tend to produce a false sag: slats that have cracked, warped, or spread too far apart to hold the middle of the mattress evenly; a box spring with broken or sunken internal support; an adjustable base panel that's lost tension over years of flexing; or a platform bed with a center support that isn't properly braced. Each of these lets the mattress dip in exactly the spot you'd expect from material fatigue — which is why they're so often mistaken for it. If the test comes back clean.

When the dip disappears on a known-flat surface, the fix is on the base side, not the mattress side — and the right next step depends on exactly what's wrong with the frame, slats, or foundation. If the test doesn't change anything.

If the same unevenness remains on a known-flat surface, the mattress itself is likely contributing to the response you're feeling. Which layer changed — and whether it's covered — is a question for the warranty terms or the brand, not something the surface can tell you. Once the base is ruled out, three outcomes on the mattress side.

The check above tells you which side to look at. If it points to the mattress, what happens next falls into one of three outcomes — and only one of them involves buying anything.

Keep — nothing here needs fixing

The middle sits level with the edges when the bed is empty. You don't drift inward during the night. Nothing about the surface gives you a specific complaint.

Nothing to do. Keep using it, and check again in about three months, or sooner if the middle starts to feel different from the edges. Support changes gradually, and the feel usually shifts before anything is visible.

No product recommended. This is the most common outcome, and it's a real one — not a placeholder before a sales pitch.

Adjust — the structure is sound, the surface isn't

The bed holds its shape under load, and the middle hasn't dropped away from the edges. What's off is the surface: it either feels too firm to settle into, or it holds heat through the night. Both are surface problems, and a topper is built for surface problems.

A topper acts on the surface, not on the structure underneath it. If the structure is intact, a topper can make that surface softer or more breathable. If it isn't, no topper will put the support back.

One trade-off worth knowing before you shop for one: soft and breathable pull against each other. The foam that creates a closer hug also reduces airflow, and a build that breathes well usually won't cradle as deeply. A topper that claims to solve both problems equally well is overstating what a single layer does. Pick the problem that affects your sleep more, and solve that one.

Replace — the middle has already lost support

A visible dip. A slope toward the center. The sense that you're being pulled inward regardless of how you lie down. A partner drifting toward you, or you toward them.

Replace it. A surface layer won't bring the structure back. Adding a topper to a mattress that has already changed underneath alters how the surface feels for a while and covers the thing that's actually happening. This is a replacement decision, not an adjustment one.
